Orthopedic surgeon performs first U.S. AI-assisted total hip replacement surgery using ORTOMA

James Germano, MD, performed the surgery on 74-year-old Dorothy Martino

NEW YORK, July 9th, 2025–(Northwell Health)–James Germano, MD, chair of orthopedics at Northwell’s Long Island Jewish Valley Stream Hospital, recently performed the first-ever artificial intelligence-assisted total hip replacement surgery in the United States using the ORTOMA platform.

On May 1, 2025, Dr. Germano performed the surgery on 74-year-old Long Islander Dorothy Martino. The Oceanside resident was diagnosed with degenerative osteoarthritis in her right hip, resulting in bone-on-bone contact that impacted her back as well. Ms. Martino says the procedure has changed her life and has allowed her to walk without the use of a cane, restoring her mobility.

What is ORTOMA?

The ORTOMA is an intraoperative guidance system used for total hip replacement procedures. The platform aids in surgical precision and shortens the duration of the operation. During the preoperative planning of a total hip replacement, a CT scan image is needed to plan the exact positioning of the implant for leg length and offset. Dr. Germano says that when patients undergo total hip replacement, the CT scan for the hip is typically sent to another location for data analytics, it can take up to two weeks to receive the results. The ORTOMA provides the CT scan data within 4 minutes.

The system also allows a surgeon to use their own equipment in the operating room with only minor alterations. Dr. Germano says that this makes it very comfortable for surgeons to transition to the device’s technology. The ORTOMA unit also has a small footprint, making workflow easier with little deviation for technique in the operating room.

“This milestone represents more than a first for our hospital; bringing the ORTOMA system to our surgical procedures is a pivotal step toward truly personalized care,” said Dr. Germano. “This technology is a glimpse into the future of orthopedics, where artificial intelligence enhances our ability to tailor procedures to each patient with great speed, accuracy and confidence than ever before.”

Dr. Germano has been working with the ORTOMA team for more than two years. Most recently, he traveled to Sweden to work with one of the design surgeons in the operating room during a total hip replacement surgery there.

“This groundbreaking procedure reflects the innovative spirit of our hospital and the extraordinary leadership of Dr. Germano,” said Jason Tan, RN, President, Long Island Jewish Valley Stream Hospital. “Being first in the nation to use the ORTOMA system not only showcases our commitment to advancing surgical care, but also brings world-class, cutting-edge technology directly to the community we serve. It’s a proud moment for our entire team.”

The ORTOMA platform is now being used as part of a pilot program at Long Island Jewish Valley Stream Hospital for total hip replacement procedures.
